11/30/2023 EnchantaVaughn Approved QMHPA greeted Ms. Broadnax upon arrival to the home. Upon arrival to the home, Ms. Broadnax appeared to be in a relaxed mood evident by her facial expressions such as a smile on the face. QMHPA engaged Ms. Broadnax in a discussion to determine how her day was going. Ms. Broadnax verbalized to t QMHPA that her day was going well; however, her boyfriend was getting on her nerves and she did not have any food in the home so she didn’t know what she was going to do about not having food. Ms. Broadnax did not present any symptoms of SI or HI at the time of the session being conducted. QMHPA began the session by inquiring about Ms. Broadnax’s current thoughts, feelings and emotions. QMHPA stressed to Ms. Broadnax the importance of informing her daughter of the things needed within the household in advance and not at when it is needed. QMHPA questioned Ms. Broadnax about taking her mediations as prescribed. QMHPA reiterated the importance of Ms. Broadnax continuing to follow a consistent medication regimen in order to allow her body and emotional state to adjust to the medications such as the effectiveness of the medication. QMHPA listed attentively as she expressed with QMHPA her increased depressed mood and inappropriate verbalization when engaging in conversation her peers. QMHPA redirected her inappropriate verbalization and aggressive communication skills several times while informing Ms. Broadnax that QMHPA understands her level of frustration in regards to the situation when there is no food in her home and when others in her family continues to abuse things in the home such as food. Ms. Broadnax appeared to be receptive to QMHPAs’ interventions and services that were provided to her during the session. Ms. Broadnax had to be redirected by QMHPA several times for utilizing profanity as she was expressive when discussing her current feeling as it relates to her not having food in the home throughout the month. During the session Ms. Broadnax was able to knowledge her behaviors and how they do have a negative effect on her overall level of functioning. Ms. Broadnax continued to verbalize to the QMHPA that she will continue to do what needs to be done in order to ensure that her and her family has food in her home to last them until she gets her food stamps (which is not much as to why she is always low in food supply).
